Head bolts are tightened in a set sequence that works outwards from the centre, in several stages, and on most modern engines the final stages are an angle rather than a torque figure. Getting that sequence or those angles wrong distorts the head and the gasket fails regardless of its quality.

What is angle tightening and why is it used?

After an initial torque the bolt is turned a further stated number of degrees, for example 90 degrees plus 90 degrees. This stretches the bolt into its yield range, where the clamping force it gives is far more consistent than a torque wrench alone can achieve, because friction in the threads no longer dominates the reading.

Can stretch bolts be reused?

No, where the manufacturer specifies them as single use. A bolt taken past its yield point has stretched permanently and will not give the same clamping force a second time. Check the length of each old bolt against the maximum in the manual: any bolt over that length is finished even if it is being reused elsewhere.

What are the signs of a failed head gasket?

White sweet-smelling smoke from the exhaust, coolant disappearing with no external leak, a creamy deposit under the oil filler cap, bubbles rising in the radiator with the engine running, and overheating that returns after topping up. Any two of these together make a head gasket the most likely cause.
