An idler pulley does not drive anything. It guides the V-ribbed belt around obstructions and increases how much of each driven pulley the belt wraps around. How do I check an idler pulley?

Remove the belt and spin the pulley by hand. It should turn freely, quietly and coast for a moment before stopping. Roughness, a grinding feel, or a squeak means the bearing has failed. Why does a failing idler damage the belt?

A bearing with play lets the pulley tilt under belt load, so the belt is pulled to one side every revolution. The belt edge frays, the backside becomes glazed, and the belt runs hot. Are all idler pulleys smooth, or do some have grooves?

Both types exist. A smooth idler runs against the flat backside of the belt. A grooved idler runs against the ribbed side and has the same 40 degree grooves as a driven pulley. Heat and load over distance, and on many engines a small sealed bearing is doing thousands of hours at high speed. Belt tension pushes on it constantly, and an over-tight belt shortens its life directly. Can a noisy idler be greased instead of replaced?

No — it is a sealed-for-life bearing and prising the seal out to add grease destroys the seal and shortens what life is left. Once an idler is noisy the raceway is already damaged and grease will not restore a pitted surface. They have run the same hours under the same tension, so one failing means the others are close behind, and the labour of reaching them is already spent. Does an idler have a direction of rotation?

The bearing does not, but the pulley face does matter: a flat idler runs on the back of the belt and a grooved one runs on the ribbed face, and they are not interchangeable.
