What is the actual damage?

Only in one narrow case: topping up oil between services is not an oil change and does not require a filter. If you are draining and refilling the sump, change the filter. How much dirty oil stays behind in the old filter?

Between 0.3 and 1.0 litres depending on the can size, and every drop of it is the oldest, dirtiest oil in the engine. What does it cost in the end?

The saving is the price of one filter. The risk is the bearings, which on a light truck diesel means a crankshaft regrind, a set of shells and a week off the road — several hundred times the filter. Is topping up oil the same as changing it?

No. Topping up replaces the oil that was burnt or lost and dilutes what is left, but it does not remove the soot, the acid and the spent additives already in the sump, and it does nothing for the filter. Is changing the filter without the oil any better?

It is the less useful half of the job. A new filter cannot clean oil that has already lost its additives and thickened with soot, and you still lose the few hundred millilitres held in the old can. Yes, every time on a crush washer. It is designed to deform once and seal against the sump face; reused, it needs extra torque to seal and that is how sump threads get stripped.
